of Child Born Abroad |

Child
born abroad can be registered to be a Malaysian at any
Malaysian Mission overseas on the conditions:
- Father of the child is a Malaysian Citizen
- Registration
is made within 10 month from the date of birth

All forms must be requested at the Consulate or download here.
To request, please provide:
Self addressed, stamped envelope.
To submit an application in-person, please make an appointment at (213)892-1238. |

Documents required are as follows:
- Parents
original passport and photocopy
- Parents
original Malaysian Identity Card and photocopy
- Parents
original Birth Certificate and photocopy
- Original
and photocopy of Malaysian Marriage Certificate
- Original
and photocopy of Child's Birth Certificate
- Copy
of Father's Green Card
- One
(1) recent passport size photographs of your child.
Please see
photograph requirements.
- Registration
fee of US$10.00 payable to Consulate General of Malaysia.
- For
the return of your passport and documents, please
complete mailing
address information and provide at least $30.00
money order, made payable to FEDEX and send
it together with your application form.
The Consulate is not responsible for
documents lost in mail.

- All applications must be signed by the child's Father Only.
- No Endorsement can be made in the Machine Readable Passport.
- All applications from Los Angeles area must be submitted in person.
- Please
submit application before the child's 10th month old birthday
to ensure on time processing.
- All late registration (past the child's 1st birthday) must be submitted personally to the National Registration Department in Malaysia to apply for TARAF KEWARGANEGARAAN only
