4. Name and Address of Mortuary
5. Original Passport of Deceased
6. A copy of passport for the person accompanying the body / ashes
7. A copy of Flight itinerary/ticket.

• United States Postal Service Express Mail only.
• Money Order of minimum $25.00 payable to FEDEX.
