Passport Services
Emergency Travel Document (EC)
 

This document is issued to Malaysians holding an expired, lost passport or passport validity less than 6 months to return to Malaysia one way via all countires except listed below.

Complete form:

IM.42-Pin1/1997
· Right Thumbprint & Signature Notarization (disregard if submit in person or new born baby)
· Registration of Malaysian Citizens Abroad
(must be completed by applicant) Malaysians who fail to register with the Consulate on a yearly basis would face the risk of having their nationality revoked under Article 25(2) of the Federal Constitution.

Application From (IM.42-Pin1/1997) Translation

 

Documents required are as follows:

  1. Current Malaysian International Passport.
  2. Original Malaysian Identity Card and a photocopy (front and back)
  3. Photocopy of Malaysian Birth Certificate / Malaysian Citizenship Certificate / Borang W (for child born abroad).
  4. Three (3) recent passport size photographs (DO NOT GLUE PHOTO ON THE FORMS).

  5. A photocopy of your Visa / Green Card / Employment Permit / I-20 (for student).
  6. Processing fee of US$17.00 in the form of POSTAL MONEY ORDER ONLY. Payable to Consulate General of Malaysia.
  7. A photocopy of confirm flight ticket.
  8. For children under 18 years old, please provide:
  • Original and a photocopy of the father's/guardian's Malaysian Identity Card
  • A photocopy of father's/guardian's valid Visa/Green Card
  • Father/guardian must sign item no. 13 (on Bapa/Penjaga yang sah) as consent to issue passport to the child and right thumb print (black ink only) next to the signature.
  • Children under three (3) years old is exempted from providing a thumb print.

For the return of your passport and documents, please complete mailing address information form and contact your chosen secure courier service for rates and additional fees. Also include necessary envelope and air bill for mailing.

Note:

  • For United States Postal Service please use Express Mail only.
  • No charge to credit card.
  • You may also choose to send us a money order in the amount of at least $25.00 made payable to FEDEX for document mail out.

The Consulate is not responsible for
documents lost in mail.

Applicants living in all five (5) Los Angeles counties (San Bernardino, Riverside, Los Angeles, Ventura and Orange) MUST SUBMIT and COLLECT documents PERSONALLY.

 

*IMPORTANT NOTE:
Please submit your application at least one month before the departure date. Processing will take approximately 5 to 10 working days.

Consular Service Hours for phone and walk-in:
9:30 am to 12:00 noon, Monday to Friday.
