Complete: Three copies of the application from and online registration (e-CONSULAR)
*E-CONSULAR
must be completed, printed and submitted together along with the 3 application form.

Documents required are as follows:

Three(3) recent Passport size photographs.
See photograph requirements.

Three(3) certified copies of all Malaysian visa stamped pages.

Three(3) certified copies of your passport namely pages that contain details of the applicant (front page and last page)
*Note: Kindly ensure that the passport has a minimum validity of nine (9) months from the date of application. If the validity is less than nine (9) months, kindly renew your passport before applying.

An introduction letter from your employer.

Money Order in the amount of US $7.00 payable to the Consulate General of Malaysia, Los Angeles (No personal check accepted).

A self-addressed return envelope with sufficient stamp.

For your information, your application has to be referred to the relevant authorities in Malaysia for necessary clearance. You will be notified by mail upon receiving clearance from Malaysia. Processing time will be within 4 to 6 months from the date of submission. For in-person or phone services the Consular Service Hour is Monday to Friday from 9:30 a.m. to 12:00 p.m. only.
