Letter of Good Conduct Application for Malaysians |

Please call the Consulate and make an appointment. You must be present to sign the documents in front of the Consular Officer.
Complete: Three copies of the application from and online registration (e-CONSULAR)
*E-CONSULAR
must be completed, printed and submitted together along with the 3 application form.

The Document require as follows:
- Three
(3) recent Passport size photographs (front view).
See photograph requirements.
- Three (3) certified copies of your Malaysian Identity Card.
- Three (3) certified copies of your Passport namely pages that contain details of the applicant (front page and last pages).
*Note: Kindly ensure that the passport has a minimum validity of nine (9) months from the date of application. If the validity is less than nine (9) months, kindly renew your passport before applying.
- Money order for USD7.00 made payable to the Consulate General of Malaysia, Los Angeles (No cash or personal check accepted).
- A self-addressed return envelope with sufficient stamp.
For your information, your application has to be referred to the relevant authorities in Malaysia for necessary clearance. You will be notified by mail once the Consulate receives the approval from Malaysia. Processing time will be within 4 to 6 months from the date of submission.
For in-person or phone services the Consular Service Hour is Monday to Friday from 9:30 a.m. to 12:00 p.m. only.
