1. Three (3) recent Passport size photographs.

2. Three (3) certified copies of your Malaysian Identity Card.
3. Three (3) certified copies of your Passport (all pages).
4. An introduction letter from your employer (for foreign nationals).
5. Barcode page issued by U.S. Department of State.
6. Money Order in the amount of US$7.00 payable to the Consulate General of Malaysia (No personal check accepted).
7. For your information, your application has to be referred to the relevant authorities in Malaysia for necessary clearance.  Once your application has been approved, it will be forwarded to Embassy of Malaysia in Washington D.C. to be submitted to the US Department of State. Processing time will be within 4 to 6 months from the date of submission.
   
   For in-person or phone services the Consular Service Hour is Monday to Friday from 9:30 a.m. to 12:00 p.m. only.
