With reference to your inquiry on the visa application, forwarding herewith form IM.47-Pin 1/97. Kindly return the completed IM. 47 - Pin. 1/97 form to this office with the following :-

1) Your current International Passport and a copy (Passport must be valid a minimum of six months prior to entry into Malaysia).

2) One (1) recent passport sized photograph (Front View).

3) Money order for processing (please call the Consulate General for the appropriate amount depending on country.)

4) A copy of your flight ticket or flight itinerary from your travel agency pertaining to your trip to Malaysia and a copy of your hotel reservation / place of stay.

5) A copy of your Resident Alien Card or Visa Status in U.S.A.

6) For Business Trip

i.) An invitation letter from the company in Malaysia pertaining your business visit.

ii.) A business letter from your current company in the U.S.

7) For Social Visit

i.) A letter of invittion from family friends/spouse in Malaysia or hotel reservation confirmation

ii.) A photocopy of family/friends Malaysian Identification Card and Passport.

8) For applicant who is married to a Malaysian Citizen, a copy of the Malaysian Marriage Certificate and copy of spouse Malaysian Passport and Malaysian Identification Card is required.

9) An employment verification letter from your current company is required.

10) For the return of your passport and documents, please complete the mailing address information and provide a separate $30.00 money order, made payable to FEDEX and enclose it together with your application form.

NOTE: Visa Application form should be submitted to this office at least 2 weeks prior to departure date. Failure to comply with the above requirement will delay the process. This office will not hold responsible for any delay to your travel arrangement. Visa will be processed within 5 working days.
