Renunciation of Malaysian Citizenship Application |

Complete form: Borang K in duplicate for your completion. |

All forms must be requested at the Consulate.
To request, please provide:
Self addressed, stamped envelope |

Documents required are as follows:
- Your
original Malaysian International Passport
- Your
original Identity Card
- Your
original Malaysian Birth Certificate (The original
Birth Certificate will be returned to you once your
renunciation is completed)
- Two
(2) recent passport size photographs.
See photograph requirement.
- Copy
of your U.S. Passport and Naturalization Certificate.
- Fee
of US$4.00 (money order ONLY) payable to Consulate
General of Malaysia.
- Enclosed
also Appendix A for your signature, Right Thumb Print
and Left Thumb Print and to be notarized by a Notary
Public if it is sent by mail.
- If
you have lost your original Malaysian Identity Card,
Malaysian Birth Certificate or both, please lodge
a Lost Property Report from your local Police Department
and send a copy of your report to this office for
processing.
- For
the return of your passport and documents, please
complete mailing
address information and provide at least US$30.00
money order, made payable to FEDEX and send
it together with your application form.
The Consulate is not responsible for
documents lost in mail.
