1. Your original current Taiwan Passport and a copy
   (Passport must be valid a minimum of six months prior to entry into Malaysia).
2. One (1) recent passport size photograph.

3. A copy of your flight itinerary from your travel agency pertaining to your trip to Malaysia.
4. A copy of your hotel reservation / place of stay.
5. A copy of your Resident Alien Card or Visa Status in U.S.A
6. A copy of your Bank Statement.
7. For Business Trip:
   - A business letter regarding the nature of business visit in Malaysia.
   - An invitation letter from the company in Malaysia.
8. For Social Visit:
   - An employment verification letter from your current company.
   - Letter of invitation from family/friend/spouse in Malaysia.
   - Photocopy of family/friend's Malaysian Identification Card.
   - If spouse is Malaysian, provide a copy of Marriage Certificate and Malaysian Identity Card.
9. Processing fee of US$17 should be remitted by MONEY ORDER ONLY and made payable to Consulate General of Malaysia. No personal checks are accepted
10. For the return of your passport, please provide a separate $30.00 money order, made payable to FEDEX

** The Consulate is not responsible for documents lost in mail.

NOTE: Visa Application form should be submitted to this office at least 2 weeks prior to departure date. Failure to comply with the above requirement will delay the process. This office will not hold responsible for any delay to your travel arrangement. Visa will be process within 5 working days.

For in-person or phone services, the Consulate Service Hour is Monday to Friday from 9:30 a.m. to 12:00 p.m. only.
