Nationals of the above who wish to apply for visa to enter Malaysia are requested to fulfill the following requirements and completed IM. 47 - Pin. 1/97 form when forwarding their application to this Consulate :-

1. Your original current passport and a copy. (PASSPORT SHOULD BE VALID A MINIMUM OF SIX MONTHS PRIOR ENTRY INTO MALAYSIA)

2. One recent passport sized photograph (2" x 2") - Front View

3. Fee for single entry must be remitted by MONEY ORDER ONLY made payable to the Consulate General of Malaysia.

Fee:  Sri Lanka      -      USD 5.00
         Pakistan        -      USD 10.00
         Bangladesh   -      USD 7.00

4. A copy of your flight ticket or flight itinerary from your travel agency pertaining to your trip to Malaysia and a copy of your hotel reservation / place of stay.

5. A copy of your Resident Alien Card or Visa Status in U.S.A.

6. For the return of your passport and documents, please complete mailing address information and provide a seperate $30.00 money order, made payable to FEDEX and enclose it together with your application form. The consulate is not responsble for documents lost in mail.

NOTE: Visa Application form should be submitted to this office at least 2 weeks prior to departure date. Failure to comply with the above requirement will delay the process. This office will not hold responsible for any delay to your travel arrangement. Visa will be process within 5 working days.
